1. What kills?

We have a written list of the fatal risks in our operation.
Each fatal risk is defined as a release of energy (e.g. fall of object), not an activity or behaviour (eg. dropped object)
Incidents are classified by the fatal risk involved.
Our leaders can name our top three fatal risks without looking them up.

2. How does it kill?

For each fatal risk, we have mapped the causes that lead to loss of control (e.g. a bowtie).
Our bowties were built with the front-line workers who do the work.
We use industry fatality data, not just our own incidents, to understand how each risk kills.
Our investigations identify which critical control failure led to the event.

3. What stops energy from killing?

Critical controls are identified for each fatal risk (typically 5 to 9 per risk).
Every listed control is a specific, measurable object or action, not training, a procedure or a PPE reminder.
Each fatal risk has one named Risk Owner accountable for its critical controls.
Each critical control has a performance standard that explicitly defines pass and fail criteria

4. Are the controls in place and effective?

Critical controls are verified in the field on a schedule, with clear pass/fail triggers.
Leaders see a report of control health (effective, weak, absent), not just counts of activities.
When a critical control fails, there is a defined response and an owner.
Incident Corrective actions investigate why controls were absent, rather than why people make mistakes.

Level 2: Risks known, controls unclear.

Next: bowties built with your workers, and controls that meet the definition of a control.

Recommended: a Sample Bowtie or Sample Incident Assessment.

In our experience, most Alberta companies are still working on question 1: What Kills. That's where the work starts.
